and [det[(Y (e1 , e2 ))T Y (e1 , e2 )]]1/2 = | det(B12 aIn 2 )|| det(B12 bIn 2 )|(a b). Step 4. Notice that B1 aIn 1 0 B12 aIn 2 0, and similarly, B2 bIn 1 0 B12 bIn 2 0, and that for a > b, under the conditioning in (13.20), there is equivalence in these relations. It is also clear that since B12 0, one has | det(B12 aIn 2 )| 1B12 aIn 2 0 a n 2 , I and it follows that the conditional expectation in (13.20) is bounded by a n 1 E | det(B12 bIn 2 )| 1B12 bIn 2 I (i = 3, . . . , n) .

Occlusal Appliances Occlusal appliances generally provide a bene cial effect for masticatory muscle pain, TMJ pain, TMJ noise, jaw mobility, and TMJ dislocation.1,2 Wearing an occlusal appliance at night generally reduces morning TMD symptoms and requires minimal effort from patients.2 There are many different types of appliances, but no one appliance appears to be signi cantly superior to another.3

We decided not to include the results for all of the model variables since the patterns for the ones omitted were similar to those observed for these four. The first thing to be observed from Table 20.1 is that the parameter estimates do vary slightly over the five estimation approaches, but that the estimates under the working independence assumption stand a bit apart from the others. This could indicate that the superpopulation marginal model is not correctly specified, so that the census parameter being estimated under working independence differs from the census parameters being estimated under the working covariance assumptions for the other approaches. This is not an unreasonable conjecture since we spent little time on diagnosing whether or not we had a good model. Table 20.1 also shows that, for our example, the standard errors are quite similar under the four methods of modelling odds ratios. Also, the gain in efficiency (in terms of SE) over SEE-Ind is quite small in this example. A possible explanation for this small efficiency gain is that the covariates are nearly cluster-specific in the sense of not changing from t = 1 to t = 2. In the case of a cluster-specific covariate, Lee, Scott and Soo (1993) and Fitzmaurice (1995) have shown that the efficiency gain over the working independence method is small. On the other hand, they have also shown that working independence can lead to a considerable loss of efficiency in estimating parameters associated with time-varying (or withincluster) covariates.
